  • Visit members with complex care needs at home (6-8 members per day).
  • Perform comprehensive health assessment which include but not limited to the following:
    • Obtain history and perform physical examination.
    • Complete review of medicines and related instructions.
    • Review home safety and psychosocial needs
    • Fulfill documentation needs for STAR and HCC measures.
    • Obtain and document all pertinent information in patient’ s charts and case management notes on same day as patient is seen. Include medications and end of life preferences and “ drivers” of preventable hospital admissions.
    • Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies.
    • Maintain patient safety and confidentiality.
    • Universal precautions utilized in all patient care.
    • Prescribe/order procedures, treatments and medications within scope of practice.
  • Evaluate patients at the Post Discharge Clinic with the attending physician.
  • Develop high level of trust and intimacy with patients and families.
  • Primary responsibility for initiating and maintaining peripheral lines, administering medications, maintaining and ordering of medications and supplies for infusion services in home, office and UC settings.
  • Perform services and procedures in all settings within NP/PA scope of practice.
  • Work in close coordination with all members of the ACO Medical Management Team, hospitalists, primary care physicians and Medical Directors.
  • Attend telephonic clinical rounds on patients under care.
  • Initiate discussions with patient and family regarding palliative and hospice care for eligible patients AND address end of life issues and obtain advance directives and POLST on all patients.
  • Provide care for patients in the skilled nursing and assisted living facilities in collaboration with the physician of record.
  • Provide patient education and participate in family and care giver teaching.
  • Other duties as assigned.

  • Assist with training and orientation of new personnel.
  • Identify problems, provide solutions and evaluate outcome.
  • Assist with the scheduling and notification of patients.
  • Participate in the planning and development of programs for High Risk patient management and education.
  • Communicate daily including weekends, with assigned supervisor.
  • Be familiar with Employer contracts with hospital facilities, urgent cares, specialists and ancillary vendors and use only preferred providers.
  • Document and maintain records within the company EMR on same day as patients are seen.
  • Other duties as assigned.

  • Providing after hours calls for patients under care 10 days a month.
  • Provide physical and /or telephonic availability to patients as assigned by Employer.
  • Other duties as assigned.

  • Valid, unrestricted Registered Nurse Practitioner/Physician Assistant License in the State of CA.
  • Active California license
  • Current CPR certification.
  • 1-3 yrs NP/PA experience with confidence to practice independently
  • Managed Care experience a plus.
  • Maintain collaboration agreement with contracted supervising physician
