Provide direct services through comprehensive evaluation, diagnosis, treatment planning and treatment
Provide procedural services within the scope of specialty training and clinical competence
Communicate appropriately, following confidentiality guidelines, with internal and external providers to ensure quality and continuity of care Participate in the education of health professions students assigned to site rotations
Medical record documentation will be in compliance with policies and procedures.
Maintain an active Pennsylvania physician license, board certification in family medicine specialty, and controlled substance registration with the Drug Enforcement Administration. Abide by the credentialing and re-credentialing policy.
Maintain participation in Medicare, Medicaid, and other third party payer programs as required by administration.
Management of chronic diseases and commonly encountered outpatient acute medical conditions.
Knowledge and competencies in understanding and collaborating with the region’s behavioral health and social service system.
Comfort providing above services to individuals with severe/persistent mental illness, substance use disorders, and housing instability.

Compensation and Benefits:
