Located 20 miles north of Charlotte, NC near Lake Norman
Full-time permanent position
Daytime Shifts
Monday – Friday
See 16-18 patients
Minimum of 6 hours to be spent at each day's scheduled facility. Once on-site work is completed, the APP must be available/on call for remote triage up until 5pm but are not required to return to either facility until the next scheduled day for rounding.
2 years of SNF experience preferred but not required
Provider is responsible for all daily sick visits, management of patient census and overall well-being of patients in the assigned facility.
Be able to take calls during day Monday – Friday; Rotating weekend call once every 6 weeks
Current and valid NP/PA license for the State of North Carolina preferred
Annual Starting Salary: between $125,000 to $135,000
Continuing Medical Education (CME) Allowance: $1,500.00 - per year
